Key Responsibilities:

• Deliver skilled nursing care to patients in their homes
• Conduct comprehensive assessments and develop personalized care plans
• Provide patient and caregiver education to support independence and healing
• Collaborate with a multidisciplinary team to ensure quality outcomes
• Maintain clear, timely clinical documentation in accordance with regulations and company policies

Qualifications:

• Current RN license valid in Virginia
• Minimum of 2 year of nursing experience (home health experience strongly preferred)
• Excellent communication and critical thinking skills
• Reliable transportation and valid driver's license
• A compassionate and professional approach to patient care
